Sometimes, a second medical opinion may be needed, and we're here to help with that. Please complete the questionnaire below, and one of our staff members will reach out to you to schedule your appointment. We will require that all medical records be received before we can schedule the consultation.

$250 - one time fee for one hour

$100/hourly afterwards

Please keep in mind that this is a consultation only. Labs, procedures, and/or imaging services will not be ordered during your office visit.
